Submission Template as an email attachment later to
awards@futstrat.com
identifying it by the same Submission Filename on the entry form below (the name of the company whose case study is being submitted).
Payment can be made later and you may
cancel your submission at any time.

contacting regarding their award or if we have any questions. Vendors may nominate their customers or user organizations can nominate
themselves.

September 24, 2010.
PAYMENT
There is no limit to the numbers of entries you may nominate, however
a non-refundable submission fee of $295.00 is required to
accompany each submission and each submission
requires a separate form.
We will pay by credit card.
Please
invoice us at the Nominator's Address above.
(You will be emailed a proforma
invoice to enable you to process payment). Upon receipt of payment your
entry will be forwarded to the judges.)
We will pay by check.
- If paying by check, please make
the check for $295 payable to
Future Strategies Inc., (the Awards Management company) in US funds drawn on a US bank and mail by express
delivery to the address below. Please include a printed copy of this
form for identification purposes.
